Print and fill out the attached Ley 300 form.
Take the completed form and the following documents with you to the Aguadilla Police Station (there is a Ley 300 window out front to turn in these documents)
- 1 passport photo (you can get them taken and printed at Walgreens)
- A copy of your Driver's License or Passport
- The original validated certificate of the Antecedentes Penales
- A copy of a recent utility bill that proves you're a resident of PR
- A legal sized envelope with a stamp on it and addressed to YFCPR's address (2700 Playuela, Aguadilla PR, 00603)
